Report a Notifiable Condition

To report non-immediately reportable conditions (24 hours a day) call (360) 740-1275 to leave a message or fax a report to (360) 740-1472. For immediately-reportable conditions during business hours call (360) 740-1223 and ask staff to connect you with Communicable Disease staff. To report immediately and 24-hour reportable conditions or a public health emergency after business hours, call (360) 740-1105 and ask staff to locate the Health Officer. If no one is available with Lewis County Public Health and condition is immediately notifiable call (877) 539-4344 to contact the Washington State Department of Health.
