Passport Applications

The Lewis County Clerk's Office is authorized by the U.S. Department of State to accept passport applications. Passport applications may be submitted, 8:00 a.m. to 4:00 p.m. Monday through Thursday and 8:00 a.m. to 11:30 a.m. on Friday.

We will attempt to process passport applications for everyone that comes into the office during the hours listed above, but the Clerk's statutory duties take precedence. Due to staffing demands and the increased volume of passport applicants we may not be able to process all applications even for those individuals that arrive during the posted hours.

To obtain a passport application, visit the Lewis County Clerk's Office or you may download a form from the U.S. Department of State.

All first time applicants and those ineligible to renew by mail must appear in person.

How Can a Passport Help You Fly Domestically?

Passport Card

The U.S. passport card is a wallet-sized, plastic passport that has no visa pages. The card is proof of U.S. citizenship and identity, and has the same length of validity as the passport book.

The card is for U.S. citizens who travel by land and sea from Canada, Mexico, Bermuda, and some Caribbean countries.

The card is not valid for international travel by air and is cheaper than the passport book.

You can apply for either the book or card, or both documents.

Passport Renewal

If it has been less than 15 years since your previous passport was issued, you may be able to submit the application by mail using form DS-82. The form contains four questions on the first page to determine eligibility.

Eligible U.S. citizens who are applying for routine service can now renew their passports online. To get started, go to our Renew Online page.

You cannot renew a passport for a child under age 16. You must apply again in person using Form DS-11.
