Identity Theft/Fraud Resources

The Lewis County Sheriff’s Office is dedicated to ensuring that our community has access to the best tools available to report and combat Identity Theft/Fraud in our communities. To that end, we would like to introduce everyone to the new IdentityTheft.gov website from the Federal Trade Commission. We are asking our community members to use this site to fill out and print an Identity Theft/Fraud information form that will assist our Office in gathering the information needed to investigate your case. You will also have the opportunity to see a list of resources that can help you recover from the ID Theft/Fraud that was perpetrated against you.

Instructions: Click on the link below to be redirected to the United States Federal Trade Commission’s IdentityTheft.gov website and click the red “Get Started” button. Answer all questions accurately and provide as much detail as possible. Once you have completed the process, you will need to print the “ID Theft Victim’s Complaint and Affidavit”. Once you have this, please write the Case Number (if you have it) and the Deputy’s name at the top and turn it in to the Sheriff’s Office.
